This learning exercise helps body-art students review the process without using real client information.

Give every field a purpose

Start with a blank copy of the form your training provider uses. Beside each field, write what decision or record it supports. If you cannot explain why a detail is requested, flag it for review. Adding more personal information does not automatically make a form more useful.

Separate the conversation from the signature

A signed page is a record of an interaction, not a substitute for explaining the proposed service and answering questions. In a practice discussion, identify what the learner should explain, what needs clarification and when to ask the instructor for help. Do not invent answers to a client’s health question.

Keep the record out of the portfolio

Use a fictional client name and invented contact details for coursework. A photograph of a completed real form can expose information even when it is only intended to show that the exercise was done. Ask your tutor how to demonstrate the task without sharing a client’s details.

Use South African privacy guidance

The Information Regulator publishes POPIA information and guidance. Use that official starting point when reviewing how a studio collects and handles personal information. Ask an appropriate adviser to check the actual form and process, including sensitive information, rather than treating a downloaded template as a complete compliance solution.

Official reference: Information Regulator: POPIA.

Bring a specific question to your tutor

Use your notes to identify one part of the process you cannot yet explain. Ask your tutor to review it before using a form with a client. These are study questions, not a ready-to-use consent form or a substitute for advice on your studio’s obligations.

Explore the courses available in your region and check the curriculum for your chosen discipline.

Good documentation supports a clear conversation and a record that the right person can understand later.
